Why - there is stigma around words such as ‘problem’, ‘issue’, ‘medical’ and ‘patient’, especially if this is available to all
Curated - in the subtle hope that it is updated regularly
Clinical - only things relevant to a person’s care should be on the list
Summary - reminder to keep it short and succinct
