Has anyone else been following the coroners inquest into baby Charlottes death and noticed the midwife comments on struggling with the EHR and documenting vital signs? I’m wondering what they are using? It sounds like some good lessons here.
Below are a couple of links. I have also been wondering what the electronic record that they were having trouble with is too.
From RNZ:
“He said he chose to proceed with a vaginal birth quickly without access to the patient’s files because the new electronic record-keeping system was not working properly.”
Another source didn’t specifically mention the electronic medical record:
“Mr Harding said the senior doctor subsequently told him he hadn’t seen all of the patient’s notes.”

Hi Ruth,
They were using the national implementation of Badgernet aka MCIS. The same system that others in NZ and England claim as being the future of obstetric informatics and the best thing since sliced bread. With our byzantine legal system, these tragic events happened several years ago and hence it was also a much earlier version of Badgernet, so we may not be considering apples with apples here.
Thanks @Greig good point, I think this was 2015? I heard some of the evidence from the midwife and her concern that she could not properly record vitals as there was a 5 minute time out and would require lengthy log in and turning her back to the patient. [quote=“Greig, post:4, topic:9258”]

Be really good to hold a digital ‘M and M’ when this case has gone through and all the evidence available?
Ted, are you using BadgerNet? May we come and visit you early in the week after the Summer School? And would you be interested on joining the rest of our crew on any of our visits that week?
@Ruth_Large, I agree. For the Midcentral team, the rollout of the national and regional programs was a steep learning curve, it would be good to both reflect and share those learnings.
I am already scheduled to discuss with the Operations Executive in charge early next week, I will raise the idea with her.
I’m shocked. What are you using in Maternity?
