- I very much agree that we need to identify the role of the person entering a ‘problem’. This can help validated if we ‘trust’ the data
- I personally don’t think adverse reactions or alerts should sit under a problem list as once you implement E-prescribing adverse reactions need to be coded specifically to enable drug to drug interaction alerts etc. Again, alerts have implications in terms of national medical warnings.
