FHIR APIs- Lab Results (Observation Resource) and Clinical Documents (Binary Resource)

I am currently working on a proof of concept phase to build two new FHIR APIs in the Northern Region. In essence we are looking to pilot (in a non production environment) and test a FHIR API that Sysmex has built to be installed over the Eclair Repository to publish labratory results.

The second part of the work is to develop a FHIR API for retrieving clinical documents (Transfer of Care, Clinical reports etc) that sit within our various repositories such as Eclair.

This would give systems ( we refer to them as API consumers) a repeatable, standardised and secure way to access data about patients in real time. This is opposed to the current approach which is strongly messaging orientated (HL7 ADT) or direct database look ups etc.

As part of this work we want to document use cases and requirements for both APIs so I am interested in the hearing from anyone in our CiLN network who might have some use cases in mind or if anyone has developed similar requirements in the past.

Here are some very high level use cases I have initially captured.

As a WellChild provider I want to view (within the organisation’s own electronic health record at the point of care), clinical reports/notes about a child that I have referred into for a specialist review so that I can provide follow up care and adjust my care plan for the child and Whanau.

At a Patient Portal provider I want to be able to give health consumers access to clinical information e.g. Labs data and Clinical Documents that are held by the DHB so that consumers have access to their own health information from accross the health sector.

As a paramedic responding to a critically ill patient I want to be able to access recent discharge summaries and lab results on the patient to ensure our emmergency management of the patient is safe e.g. medications administered

And within the DHB we have a specific use case in mind for the implementation of our
As an oncologist prescribing chemotherapy protocols to a patient I want the latest lab results for the patient imbeded into the system to drive automated clinical decision support regarding medication dosing.

Hi Anna-Marie,
We having been trying to surface lab results in our ED clinical management system (currently a version of EDaaG) for several years now, but eclair was missing the API to do this.
Currently, when we want to see if a lab result is back (to enable to decision to made, eg discharge), we have to drill down into the patient record to eclair to check. Then if it is not there, go back and check in another half hour. Significant time waited! If we can just display lab result status next to the patient on the main screen, then we would instantly know when labs are back. Even better if we could view and sign-off at the same time too! I believe NMDHB were looking for the same too. It would be great to hear how you get on with Sysmex on this.
