Electronic death docs

Hi all, the latest update looks really good on mobile. Anyone using it yet?

I wasn’t familiar with this. Very cool. Have many people who would find this helpful. Would be keen to hear wider feedback also :slight_smile:

Our issue as a DHB is going to be about changing process as the physical act of gathering the paper work from admin kicks off the process. This form will really streamline things and I think a great opportunity to review work flow.

Interesting – two factor authentication now – default is google authentication app vs text

Then consent to check my details against the HPI – nice

NHI now shows data on file – nice

It’s still “sex” and it still only has the three options of male, female and indeterminate – hmmmmm

But yes – flows nicely and some nice look up functions now

Now how to make it launch from the main “clinical workstation” but that concerto/clinical portal or something else

Use it regularly now for 12 months but on desktop. Never considered the mobile version . Will have a look and comment

@lara will you have the same issue we have in terms of moving from paper? For us the physical act of fetching the paper work sets off a change of notifications. It’s going to be interesting to unpick.

Yes not in keeping with stats NZ! Although I think in keeping with birth cert.

Well I haven’t used it but I have seen it progress. Fine for secondary care where I believe HCS / Concerto integration is planned and there may not be the data in a format it can be downloaded but in GP care it would be best to be fully integrated with GP PMS’s.
There is a gold mine of classification data which it could capture and one click access from a GP PMS would open the pipe for other conditions not related to death to be populated as data (Read codes).
Not sure it declares the patient dead to NES which would be handy?
Getting forms to funeral directors has never been my priority as they have that in hand.
We are going to create an ERMS form which will meet the above data requirement and be available as a one click form including cremation and death cert. ERMS is the south island electronic Request / Referal Management system.

Made a few inquiries via our hospital, and it seems that most of the young doctors and registrars are using it regularly. And they do the majority of these forms any way.
So we are good in the Taranaki DHB

we been looking at re-building our discharge summary in CWS, and making a link from that should be doable. Couldn’t make it work well via SMT. i.e. it as available as a kind clunky link which is not in context.

One of the main challenges we found re usability was helping users navigate the confusing paperwork is actually not a lot easier yet. The principle challenge is that the COR28 (hospital record of death) is not part of the death docs document suite… this document is used to screen those deaths appropriate for coronial referral and is used as part of the notification to the coroner where required.
i.e. Decisions about what paperwork should be completed are dependant on the need for coronial referral.

I expect we will need to replicate the COR28 inhouse as part of a deceased discharge summary. I haven’t looked for a while, but incontext link (i.e. From Mr Smith’s deceased discharge summary, take me straight to completing a death cert and cremation cert for Mr Smith) wasn’t possible yet.
