A telehealthcare day

I woke with a cold this morning.

That was a hassle. First patients booked for 830. Pre 2019 you’d just go to work, try not to sneeze on anyone, remember the handwashing and that would be that. 2021 is a bit different.

Teams message to all staff that I wouldn’t be in, but would work from home.

Run a query in Evolution to select all my patients for the day and send them all an SMS saying that I’d call or videocall them, but to text back if they want to cancel.

Pop in to work, just up the road, and have my nasopharynx ticked with a plastic swab… back home.

Start consulting - 3/4 were video, 1/4 phone of those who had not cancelled. (Only 3 cancelled).

Its easy and effective doing this from home. The integration of the PMS with other services makes it all so easy.

Primary IT for the hosting, the security, the support, the remote access
ManageMyHealth and Vino Ramayah for the video consulting platform, the booking, the portal where the patients sent the photos for me to review, and where I could send them links after the consult… Also for the direct referral using Specialist Referrals and to Pacific Radiology. And the link to SCL labs into Sysmex Asia Pacific database for lab ordering and viewing
DrInfo for the great reScript tool for sending from PMS to NZePS and the pharmacies
HealthLink for our secure connected care connections
BPAC for the cardiovascular risk assessments
Ken Leech and Procon for the point of care patient dashboard
Dragon NaturallySpeaking for the legible notes and voice controlled actions n the PMS
And last but by no means least to Geoffrey Sayer and Medtech for tying it all together with Evolution

Those are the main tools I use every day, some to a greater or lesser extent. There are more, but these are the key ones. I’m looking forward to Hira, but lets not pretend that we don’t already have great connections between multiple systems that let a busy GP do the job that needs to be done!

Thank you for the info on telehealth and a Happy New Year to all participants in this eHealth Forum!!

At last I see a systems structure diagram that links the various components of the telehealth environment. Hopefully we will see more such diagrams as Hira is developed?

This is my first post to the eHealth Forum as a healthcare consumer. I am still waiting for online access to my EHRs. I believe that Orion Health’s Health One system is available to all in the South Island. It would be great if the SI PHOs arranged for consumers’ online access to their EHRs?

Great to have you on the eHealth Forum, Geoff! The consumer perspective (except that of our own consumer experiences) is often missed by us who work in digital health, and we value your thoughts.

Mostly; in my opinion it is a real strength of the SI clinical informatics scene. In Dunedin, all GPs/PHOs and the DHB have access to it and some private clinicians do as well. However, there are many private medical / surgical specialists who do not unless they access it via the DHB system, and I don’t believe that Mercy Dunedin (the major private healthcare provider) does.

It certainly would. With my consumer :cowboy_hat_face: on, I am quite frustrated by the very limited access to my clinical record provided by my GP and the complete lack of one provided by the DHB.

I thought the attached Telehealth Concept Diagram that I made when involved in the early NZ Telehealth Forum may be of interest?

It would be great to have some diagrams, like that drawn by Richard, with brief descriptions of the IT interfaces and applicable standards.

Great example of how technology can build capacity to better meet patients needs regardless of circumstance. 4 patients reviewed and treated who otherwise would have had to wait and add to future demand. Well done you and glad it was just a cold. Hopefully our current pandemic virus will be just a cold in a few more mutations.
