The future of referral management

I’m mindful that clinicians (docs and nurse practitioners) working in an analogue environment that relies on patient narratives can cause issues when converting the narrative to structured data in a referral form. Poor quality of referrals (incomplete content, wrong content, wrong recipient) was one of the major motivators for initially creating the e-referral system. Has the use of e-referrals instead caused new ways to do them badly?
